to provide transportation for my son/daughter for the Gearhart Cup trip. This includes travel from Jefferson Park to Seaside/Gearhart, OR on Tuesday, August 1, with departure time at approximately 8:00am, some limited travel between Seaside & Gearhart between August 1 and August 4, and return trip from Seaside to Jefferson Park on Friday, August 4th, returning at approximately 9:00pm.
Agreement
I understand that transportation, activities, and accommodations for the Gearhart Cup take place outside of regular First Tee programming hours, and at a remote location.
While organized and led by First Tee employees, and with a strong emphasis on the safety and health of participants, this is a special arrangement and both the First Tee organization and the abovementioned individual (driver) is absolved from liability.
I assume all risks of injury whatsoever and agree to hold harmless the First Tee Chapter and headquarters Office, as well as the driver personally from claims(s) of any nature arising from this arrangement, including transportation.
